Content limits

1	title 1–2 行，每行 ≤ 6 字（字号 128u）
2	lead ≤ 44 字
3	contents 2–4 项，每项 ≤ 12 字，写本章各页的标题
4	icon 选一个能代表整章的图标，自动用强调色
5	没有 source 字段
